Declaration of participation in rural endowment insurance

Those who meet the provisions of Article 3 of the implementation opinions shall apply for the new agricultural insurance at the service center of the place where the household registration is located. Select the level of payment, fill in the Registration form of New Rural Social Endowment Insurance, sign the Agreement on entrusting Banks to withhold Rural Social Insurance premiums (hereinafter referred to as "withholding Agreement"), and provide the following materials:

1. The original and photocopy of the resident ID card (the second generation resident ID card should be photocopied on both sides).

2. The original and photocopy of the household registration book (home page, personal page).

3. If the original household registration book cannot be provided and the category of "agricultural household registration" cannot be determined, the original household registration certificate issued by the police station at the place where the household registration is located shall be provided.

4. The original and photocopy of the designated bank's "real-name" bank card (passbook) (hereinafter referred to as the bank card).

Note: if the applicant is unable to handle it in person due to special circumstances, he or she may entrust relatives or village co-organizers to go through the declaration formalities on his behalf. When the agent goes to the service center, the trustee shall issue the "power of attorney of the new rural social endowment insurance agency" or the original power of attorney that meets the requirements (hereinafter referred to as the "power of attorney"), as well as the original and photocopy of the trustee's resident ID card.

Procedures for handling rural old-age insurance

1. The service center is responsible for the preliminary examination of the applicant's eligibility for insurance. After passing the preliminary examination, the service center shall submit copies of the registration application form, the withholding agreement and relevant materials to the district and county agricultural insurance centers.

2. The district and county agricultural insurance centers are responsible for reviewing the relevant information of the applicant, and if they meet the requirements, they shall be handled in accordance with the following procedures:

① initializes personal information in the common basic information management of the bureau information system.

② registers individuals in Shanghai's new rural social endowment insurance information management system (hereinafter referred to as the agricultural insurance system) and inputs the relevant information of the registration application form into the agricultural insurance system.

③ prints "Registration approval form for participation in New Rural Social Endowment Insurance" (hereinafter referred to as "Registration approval form"), which is sealed and handed over to the applicant by the township service center.

④ sets up personal accounts for applicants in the agricultural insurance system.

⑤ records the payment standard in the agricultural insurance system.
